- Offer available to new patients of Sandgate Family Dental only.
- Patients without private dental health cover will be charged a capped fee of $199 (GST-inclusive).
- Patients with private dental health cover will be charged their fund’s scheduled fee for the same items. Any difference between the rebate and that scheduled fee is payable by the patient.
- Health-fund card must be presented at the appointment to process claiming.
- Eligibility, rebates and out-of-pocket amounts depend on each patient’s health fund policy, level of cover and remaining limits.
